چکیده

A 49-year-old man with a history of type 2 diabetes mellitus, chronic renal failure, and sigmoid adenocarcinoma underwent a laparoscopic left hemicolectomy. A second surgery was necessary after unsuccessful endoscopic dilation of an anastomotic stricture located 12cm from the anus. After five further surgical procedures for different complications, a repeat anastomosis was necessary at 5cm from the anus. However, a stricture developed at this anastomosis and could not be resolved by traditional endoscopic dilation. The patient was referred to CIDMA (Centro de Innovaciones Digestivas Martínez Alcala) for endoscopic treatment. During endoscopy, a severe, filiform, eccentric stenosis was observed at 5cm from the anus. Successive dilations were performed using a through-the-scope, over-the-wire balloon under fluoroscopic guidance. Despite this aggressive protocol, the stenosis remained tight, with a diameter of <10mm. Therefore, placement of a novel, fully covered, self-expanding metal stent (SEMS) was proposed (AXIOS AXS15-10; Xlumena Inc., Mountain View, California, USA) (●" Video 1). The stent was placed under endoscopic and radiologic guidance (●" Fig.1,●" Fig.2). The prosthesis was well tolerated without complications or the need for analgesia (●" Fig.3), and was removed 40 days later (●" Fig.4,●" Video1). The patient remained asymptomatic and with adequate intestinal transit after 2months’ follow-up. SEMS are not widely used for benign colon disease [1]. Risks associated with the use of SEMS include perforation, chronic abdominal pain, migration or late prosthesis obstruction. Biodegradable stents are an alternative and their placement is an effective treatment in anastomotic strictures of the colon, as they are more flexible than metal or plastic stents and do not require removal because they disintegrate after 11–12weeks [2]. However, these stents are wider, longer, and thus poorly tolerated, especially in locations close to the anus. The patient presented in this report had a complex, severe, and refractory anastomotic stenosis, located very close to the anus, and therefore was not a candidate for treatment with currently available prostheses. The AXIOS echo-endoscopic stent, which was designed for treatment of pancreatic pseudocysts, is a completely covered SEMS with proximal and distal Fig.1 a,b Placement of a fully covered, self-expanding, metal stent under radiologic vision, for treatment of a benign anastomotic rectal stricture.

Endoscopic Management of Refractory Benign Colorectal Strictures

In colonoscopic study, benign colorectal strictures with or without symptomatic pain are not rarely encountered. Benign colorectal stricture can be caused by a number of problems, such as anastomotic stricture after surgery, inflammatory bowel disease, postendoscopic submucosal dissection, diverticular disease, ischemic colitis, and so on. There are various modalities for the management of beni...

Lumen-apposing covered self-expanding metal stent for management of benign gastrointestinal strictures

BACKGROUND AND AIMS Self-expanding metal stents (SEMS) are safe and effective for endoscopic management of malignant gastrointestinal strictures, but there is limited experience with their use in refractory benign strictures. We assessed the use of a new lumen-apposing covered SEMS for the management of benign gastrointestinal strictures. METHODS A single-center case-series of five patients w...

[Benign anastomotic strictures after oncologic rectal cancer surgery. Results of treatment with hydrostatic dilation].

INTRODUCTION Benign anastomotic strictures after rectal cancer surgery are common and their treatment can vary from conservative measures to surgical resection. PATIENTS AND METHODS Between March 2001 and August 2008, 422 patients with rectal cancer underwent anterior resection and 83.8% were treated with primary anastomosis. Anastomotic stricture has been defined as the inability to pass a c...

Endoscopic electrocautery incision therapy for benign lower gastrointestinal tract anastomotic strictures

Benign anastomotic strictures can occur in up to 22% of patients who undergo colonic or rectal resection. Traditionally, surgery was the preferred method of treatment, but, over time, endoscopic techniques, such as balloon dilation, have become the preferred modality. However, a high stricture recurrence rate of up to 18-20% and the increased risk of perforation due to uncontrolled stretching a...

Pericarditis: a rare complication of fully covered self-expandable metallic stent in postoperative benign anastomotic stricture.

Benign esophageal strictures are traditionally treated by endoscopic dilation with bougies or balloons [1,2]. Fully covered, self-expandable, metallic stents (SEMS) have been used in the treatment of benign esophageal disease, with the benefits of removability and low incidence of tissue hyperplasia [3]. However, significant complications, such as stent migration, recurrent stricture, or erosio...
